4/22/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C conducted an individual session with Berando. The focus of the session was to address Berando’s social behavior. Upon arrival at the home, Berando presented with positive affect, evident by his facial expression. Mother reports that Berando went into the store and stole a toy and tried to make it seem as though it was given to him by his grandmother. QMHP-C engaged in a discussion to determine why he would go into the store and steal a toy when he could have asked his mother about buying him the toy. Berando reported to QMHP-C he does not know why he did it but he did apologize to his mother for what he did. 2. Behavior: Berando appeared to be irritable at times within the session as his mother was discussing his behaviors and how he fails to accept responsibility for the behaviors he is exhibiting when he was stealing from the store. He reported to QMHP-C he was just trying to get the toy and he did not want to get in trouble. 3. Behavior: Berando appeared to be cooperative throughout the session as he was able to discuss with QMHP-C about his behaviors and how he continues to make his mother upset about the behaviors and decisions he chose to make. 1.Intervention: QMHP-C began session by processing with Berando his current emotional state. QMHP-C reviewed interventions from the previous session and explored with Berando his behaviors while in the community and with his family and peers. QMHP-C encouraged Berando to utilize positive social skills to include refraining from physical and verbal aggression with others and recognizing boundaries. 2. Intervention: QMHP-C explored with Berando any current instances of stealing and encouraged Berando to respect the property of others. QMHP-C engaged Berando in a therapeutic game and encouraged Berando to verbalize the triggers to his inappropriate behaviors. QMHP-C ended session by reviewing interventions with Berando and his mother and encouraging them to provide feedback on the session. 3. Intervention: QMHP-C encouraged Berando mother to maintain a consistent medication regime for Berando in order to address his impulsive, hyperactive, inattentive and depressive symptoms. QMHP-C ended session by reviewing interventions with Berando and his mother and encouraging them to provide feedback on the session. 1.Response: Berando actively participated throughout the session. While processing with Berando his current emotional state, Berando shared that he was in an irritated mood. While exploring Berando’s behaviors in the community, Berando shared that he enjoys being in the community and meeting new people. Berando shared that he does not engage in physical altercations outside of altercations with his siblings. 2.Response: While exploring stealing, Berando denied any stealing behaviors. Berando actively participated in the therapeutic game and shared that he wants people to recognize positives in him instead of negatives and that this was one of his triggers. 3.Response: During the parental session, Berando mother stated that she continues to be inconsistent with Berando medication compliance. At the end of the session, Berando mother shared that it continues to be difficult addressing the different behaviors for Berando and as a single mother and that she was seeking additional support.
